VISA CHECK OFF SHEET

  1. Two copies of valid passport
  2. 4 recent passport pictures
  3. Medical certificate including HIV and Hepatitis-B test:
    1. This is a letter/certificate from your family doctor as to your health status. Please include test result documents for HIV and Hepatitis-B testing.
    2. These tests must be done no more than two months before going to Bonaire.
    3. All original lab paperwork is required, and must be signed by your physician.
  4. Birth certificate (legalized with Apostille)
    1. An Apostille is a seal placed on an official document; the Apostille is widely accepted outside of the US, and is issued by the Secretary of State's office in each state (see attached SOS listing by state).
    2. If you are outside of the US, you need to contact the Dutch Embassy in your country of decent/origin to obtain the seal of Apostille. If you are born outside the US, your birthcertificate must be legalized by the Dutch Embassy and the Ministry of External Affairs from your country of birth.
    3. If you were born either in the United States or in Canada, your original birth certificate needs to be apostilled by the Secretary of State office in the state in which you were born.
    4. Your birth certificate should be legalized and not be older than one year after they apostilled it
  5. Certificate of good conduct (police record)
    1. A certificate of good conduct is obtained by contacting your local Police Department. Request them to conduct a criminal background check. Please send the printed report to us so that we may forward it with your immigration documents.
    2. This certificate should be completed no more than two months before going to Bonaire.
  6. Proof of civil status (legalized with Apostille). This document cannot be older than 6 months.
    1. This is a statement of marital/civilian status. Follow the same procedures for the Apostille seal.
    2. The Civil Status needs to be legalized by the Dutch Embassy and Ministry of Foreign Affairs only if you are not residing in the USA.
  7. Student Declaration
    1. You must complete and send only the Dutch version; the translated version is for your reference only.

  8. Guarantee Declaration
    1. You must complete and send only the Dutch version; the translated version is for your reference only.
    2. Please complete only the second half of this document; the first half will be completed by our staff in Bonaire.

  9. Bank Declaration
    1. If you are utilizing financial aid, please contact our office in Youngstown, Ohio, and request a Financial Aid Statement Letter.
    2. If you are not utilizing financial aid, please have your guarantor (parents) write a statement indicating that they support you every month. Click here for an example letter to you which states that you are in good financial standing.
  10. Model- I
    1. You must complete and send only the Dutch version; the translated version is for your reference only.

Married students cannot bring their family with them to live on the island. However, family members can stay as tourists for a maximum of three months.

Additional information and forms will be provided upon acceptance into the university.

Process for Canadian Citizens
The following steps are to be taken if you are a Canadian citizen.

    1. First you must have all your documents signed and sealed by your lawyer (notarized).
    2. Next take those documents to 77 Grenville Ave. or Road 9th Floor, Document Services. They will charge $16cdn. (Downtown Toronto)
    3. Next you must go to the Royal Dutch Embassy, 1 Dundas St. W, Suite 2106, Toronto, ON. They will only seal documents after you go to document services and get their seal. They charge $42cdn.

Process For Citizens and Residents of all other countries
Information for getting documents Apostille. These are the steps you need to do prior to go to these following offices.

    1. Please get the documents attested by the Department or Ministry of Foreign Affairs in your country.
    2. Take the attested documents and Dutch Consulate and they will Apostille (legalize) it for you.
Note: All foreign documents must be accompanied with a certified (notarized) English translation version.
